Application Checklist

Complete and submit the following documents to the St. Joseph Admissions Office by the February 3, 2012 deadline:

• Apply online or download Application and Student Information 2012-2013 form.
• $60 application fee (non-refundable).
• Forward the applicable Confidential Teacher Evaluation form to current teacher.
• Forward Parent Participation form to current school (preschool or private schools only).
• Copy of most recent report card (entering grades 1-8).
• Copy of most recent standardized test scores (entering grades 4-8).
• St. Joseph or Rainbow Parish Verification.
• Copy of Certificate of Baptism (entering grades K-2).

Applications received by February 3, 2012 deadline will be considered in our first round of admission decisions in March. Notification letters will be mailed on March 15, 2012. Late applications are reviewed on a rolling basis.
