APPLICATiON CHECKLIST

Submit completed application by the January 30, 2009 deadline:

• Complete Application 2009-2010 (front and back)
• Complete Student Questionnaire by Parent/Guardian (yellow form)
• Submit recent report card (grades 1-8)
• Submit most recent standardized test scores (grades 2-8 only)
• $60 application fee (non-refundable)
• Forward Teacher Evaluation form to current teacher – must be received by
January 30, 2009
• Blue Form is for grades 1-8; and
• Pink form is for kindergarten applications
• Forward Parent Participation form to current school (if your child is in preschool
or attends private school) – must be received by January 30, 2009.


Other application requirements also due by the January 30th deadline:

• Verification of active parish affiliation (if you wish to have parishioner status,
you must take the original application form to the parish office for
verification)
• Copy of Baptismal Certificate (Catholics only)

Financial Aid Applications:
• Completed financial aid application forms are due no later than
February 12, 2008

Applications received by the January 30th deadline will be considered in our first round of admission decisions in late March. Notification letters will be mailed on March 20, 2009.

Late applications are reviewed on a rolling basis depending on space availability.

Recommendations for potential parents:

• Attend General Information Session
• Attend Financial and Volunteer Commitment Meeting
• Attend one of the school open houses
• Attend Kindergarten Night
